The revised Florida applications have eliminated the foreign travel question; the new forms should be used going forward.
Updated Texas Summary of Premium Provisions Form (O.NB.F.B/PR.G)
The "Plan Name" line has been removed and the updated form is available through FastForms. Both the new and old forms will be accepted with the application. |
